Consent & Intake Form
For ethics and tailored support
Please complete the Consent and Intake Form, which can be easily filled out on your phone or computer. Once submitted, kindly notify your counsellor.

Invoice
Secure your session
An invoice with payment details will be sent to you. Please note that payment is required in advance to secure your session. Once payment has been made, kindly inform the counsellor.
EFT
Fees, Payments & Medical Aid
Claim form
This is a cash-based practice, which means all sessions must be paid for in advance via EFT (Electronic Funds Transfer) in order to confirm your booking. Following your session(s), I can provide you with a medical aid claim form for potential reimbursement. Please note, however, that it remains the client’s responsibility to liaise with their medical aid provider and submit any claims.

Scope of practice
Coverage for counselling services varies depending on your medical aid plan. Some schemes may reimburse sessions with a Registered Counsellor from your available savings only, and not from Prescribed Minimum Benefits (PMBs). It is recommended that you confirm the details with your provider in advance. When contacting your medical aid provider, you may ask if the following services are covered under your plan.
